Make a Request


We want to help build stronger communities and we need your help to do it.   TDS supports 501(c)(3) organizations through charitable giving, as well as local school and community projects with our community development marketing funds.


Please fill out the following form and submit it to us using the “submit button” below.    A representative will connect with you within 2-3 weeks via Email.  You will be emailed back a response, and if your request is granted you will be asked to provide documentation for your 501(c)(3) status-W9 form (if applicable), and/or other verification information.


Please understand that we get a lot of requests and we take them in the order they are received.  Your request will be reviewed by your local Market Manager and/or members of the Connecting with our Communities employee committee.   Submitting your request form does not imply a contract for service.
